Please call us at 512-458-1922. We offer appointments Monday through Friday from 9:00 AM to 4:00 PM. When you call, be prepared to tell us your complete demographic and insurance information, the doctor's name who referred you, and authorization number if your insurance requires it.
We ask that you follow these steps before you come to your appointment to help make sure things go quickly and smoothly.
- Read and sign our office policy.
- If you are a new patient or have not seen us in awhile, fill out the medical history form.
- Our HIPPA Policy is available for you to read. You do NOT have to print this form out. It is available here for you to review. You will be asked to sign a form saying you have had the opportunity to read this once you are in the office.
- In most cases, we have already received your Patient Registration information from you over the phone. If not, we will get this from you in the office.
- Always bring your glasses, contacts and solution to every office visit.
- Any old copies of prescriptions or any relevant medical records are always helpful.
- If required, obtain any pre-authorization or referral numbers from your insurance company.
- Get

If this is a true emergency, please call 911 or go to the nearest emergency room. If this problem can wait a while or can be handled with a phone call, then call our office at 512-458-1922 and listen for instructions to page the doctor on call.

Call at least 24-hours before your scheduled appointment to cancel or reschedule to another time. Otherwise, you may be charged a $25.00 missed appointment fee.
